What is Cash-Based or Out-Of-Network Physical Therapy?

If you’ve ever felt frustrated by the limits of insurance when it comes to your health care, you’re not alone. Many people want more one-on-one time with their provider and fewer restrictions. That’s where cash-based physical therapy (sometimes called direct pay or out-of-network physical therapy) comes in.

In a cash-based or out-of-network treatment model, the physical therapist enters into an agreement directly with the patient to provide services (not with the insurance company). The patient pays at the time of service. In most cases, the insurance company is not billed directly. Instead, the patient receives a detailed superbill that can be submitted to their insurance company for possible reimbursement. This paperwork specifies that reimbursement should be sent directly to the patient.

As a courtesy, some out-of-network physical therapy clinics will submit claims to your insurance company on your behalf, while reimbursement is still sent directly to you. At Core Physical Therapy DC, we are happy to provide this service for our patients.

What Does “Cash-Based” (Out of network) Physical Therapy Mean?

Despite the name, cash-based physical therapy doesn’t require you to pay in literal cash. It simply means you pay your provider directly, rather than going through insurance first. Payment can be made with credit card or HSA/FSA funds.

By removing the middleman, you gain more flexibility, transparency, and personalized care. That means:

  • The insurance company does not dictate how many visits you’re allowed.

  • You aren’t restricted to treating only one body part at a time.

  • Your care is guided by your goals, not by insurance limitations.

Benefits of Cash-Based (Out of network) Physical Therapy

Choosing a cash-based model offers several important advantages:

  • One-on-One Expert Care – You’ll always work directly with a licensed Doctor of Physical Therapy, never with PTAs, techs, or trainers.

  • Longer Sessions – More dedicated time with your physical therapist, without feeling rushed or interrupted.

  • Personalized Treatment Plans – Your care is tailored to your unique goals, whether you’re recovering from an injury, building strength, or preventing future problems.

  • No Insurance Hassles – Skip the referrals, prior authorizations, and surprise denials.

  • Transparent Pricing – You’ll always know the cost of your sessions upfront.

  • Beyond Injury Care – Insurance typically only covers what’s “medically necessary.” With cash-based PT, we can also focus on prevention, wellness, and performance.

  • Treating the Whole You – Insurance often limits care to one body part at a time. With out of netowrk PT, we can address multiple areas in the same session, helping you recover faster with a more holistic, efficient approach.

How Does Insurance Fit In?

Even though insurance isn’t billed directly, many patients can still use their benefits:

  • Superbill Reimbursement: You can submit documentation to your insurance company for possible out-of-network reimbursement.

  • HSA/FSA Accounts: Cash-based PT is typically an eligible expense.

  • At Core Physical Therapy DC: You will still pay at the time of your service, but as a courtesy, we will submit to your insurance on your behalf and you will be reimbursed directly from your insurance company.

Who Should Consider Cash-Based Physical Therapy?

Cash-based physical therapy is a great fit for:

  • Busy professionals who want efficient, high-quality care.

  • Athletes or active adults looking to optimize performance.

  • Anyone frustrated by limited coverage or denied visits.

  • People who value personalized, uninterrupted time with their PT.

Will I Get Reimbursed for Out-of-Network PT Services? Most of the time, yes! Many insurance companies (except Medicare, Medicaid, and HMOs) offer some reimbursement for out-of-network physical therapy. That means you can choose the therapist you want to work with, even if they aren’t in your insurance network.

While it may not be fully covered, patients often find the benefits are worth it with longer sessions, more personal attention, quicker results, and care that’s tailored to you instead of being limited by insurance rules.

The Bottom Line

Cash-based (out of network) physical therapy gives you more control over your health. By investing directly in your care, you receive personalized attention, longer sessions, and treatment focused on what matters most to you.
